  • Sangguniang Panlalawigan/Bayan/Barangay Resolution authorizing the Local Chief Executive to sign the Deed of Undertaking and receive assistance.
  • Photocopy of a government-issued ID of the requesting party.
  • Data from the NDRRMC, PDRRMC, DSWD – DROMIC, or similar bodies.

For Health Facilities:

  • DOH Situational Report on Emerging Diseases/Cases.
  • Hospital Profile.
  • DOH License to Operate.
  • Other relevant documents as required.
