4. Fill in all required patient information and details accurately.
5. Upload the scanned documents to the system as part of your application.
6. Wait for email notification regarding the status of your request within 24 hours. If approved, you will receive a Guarantee Letter (GL) and the MAP application form.
7. Print the Guarantee Letter and MAP application form, then submit these along with the complete documentary requirements to the designated Hospital, Dialysis Center, partner health facility, or Diagnostic Center.
8. Receive a notification about the status of your request via email within 24 hours after submission.

Applications are open Monday to Friday, starting from 8:00 am until the daily limit is reached. Unsuccessful applications can be resubmitted on the next business day.
