Requirements

- Referral letter from a referring party (not needed for walk-ins)
- Social case study report from the local government unit
- Barangay clearance
- Medical certificate with chest x-ray, complete blood count, and urinalysis results
- School records
- PWD ID (if available)
- Recent photos (one 3R size and two 1×1 size)
- Recent psychological records (for PWID)
- Release paper (for released prisoners)
